Lip Flip v. Lip Filler: What’s the Difference?

Overview

Patients walk into my room asking for a “lip flip” or “lip filler” almost interchangeably. They aren’t the same procedure; they don’t produce the same result, and they aren’t for the same person. Knowing which one fits what you actually want is the difference between a result you’re happy with and a result you’re going to ask me to fix. The choice between a lip flip and lip filler starts with one question: do you want more lip, or more of the lip you already have?

What a lip flip actually does

A lip flip is a small dose of Botox placed in the muscle that pulls your upper lip down when you smile. Relaxing that muscle lets the pink part of your lip roll outward slightly, giving the appearance of a fuller upper lip without adding any volume. The lip isn’t bigger. It’s just sitting differently.

It’s typically four to six units of Botox, takes about three minutes, and shows up in five to seven days. Results last about eight to ten weeks, which is shorter than Botox elsewhere on the face because the lip muscle moves constantly.

What lip filler actually does

Lip filler adds volume. It’s a hyaluronic acid product injected directly into the lip tissue to change the shape, size, structure, or symmetry of the lip itself. The result is visible immediately and lasts six to twelve months, depending on the product, the dose, and how your body metabolizes it.

The lip is physically bigger after filler. The goal is determined during the consultation, not by the syringe.

Who each one is for

A lip flip is the right call when your lips are a size you’re happy with, but they curl inward or get thin when you smile or talk. It’s also a good first step for patients who are nervous about filler and want to see what a subtle change looks like before committing to a longer-lasting product.

Lip filler is the right call when you want more lip, a different lip shape, or asymmetry correction. It’s also the better choice if your top lip is genuinely small at rest, because no amount of muscle relaxation will create volume that isn’t there.

These procedures can be done together. Filler builds the structure; the flip refines how it presents when you smile.

What to ask at your consultation

Bring photos of yourself smiling, talking, and at rest. Ask your provider to tell you which of the two would address what you’re actually unhappy with, and why. A provider who recommends the flip when you come in for filler, or filler when you come in for a flip, is one paying attention to what you need rather than what you booked.
