We ask for documents that confirm your providers are licensed, your clinic is compliant with regulations, and that you uphold the highest standards of care. If you already hold external accreditations, these can also be submitted to strengthen your application.
Required documents may include:
- Copies of valid medical licenses for providers
- Records showing compliance with health and workplace regulations
- Malpractice history (showing no recent judgments or settlements)
- Accreditation certificates from recognized healthcare bodies
Patient Experience
